Please provide us with a brief message that contains all of the following: (a) your name, address and telephone number; (b) the name(s) of any entity or entities with which you are affiliated; (c) a brief description of the general nature of the services in which you are interested; (d) the name(s) of an individual or entity that you believe may be adverse to you in connection with the matter as to which you require services; and (e) information as to how you learned about the firm, including the name(s) of any referral sources.
This information will allow us to conduct a check for potential conflicts of interest and to determine whether we are available to discuss the possibility of providing services to you. Please let us know if for any reason you need to cancel or reschedule your appointment.
Prior to your scheduled initial consultation with our attorney, you will receive our Tax Questionnaire via email. Please complete this form in detail and bring it to your consultation appointment with other relevant documents.
NOTE: Although completion of our Firm’s Questionnaire or Contact Us section is covered by attorney-client confidentiality privilege, it does not establish an attorney-client relationship. Accordingly, you should not transmit any information by email or fax that you consider to be confidential until you have established an attorney-client relationship with Guzhva Law Firm, PLLC (by valid representation agreement).
During the initial consultation with our attorney, a client is expected to fully disclose the entire truth (in confidence) about his or her case. All information provided by the client during the initial consultation is covered by the attorney-client confidentiality privilege.
Please be fully prepared to discuss your problem and, if necessary, have a translator available. Be sure that you provide us with all relevant documents so that they are available at the time of your consultation. At the conclusion of the consultation, Ms. Guzhva will provide a description of what professional services, if any, can be undertaken on the client’s behalf and a preliminary estimate of what fees will be incurred. If the proposal is acceptable to the client, the firm enters into a written representation agreement with the client specifying the terms of the representation.
As part of the attorney professional responsibilities, the Firm occasionally represents indigent clients, on a pro bono basis. Attorney Olga Guzhva fully recognizes her obligations to all her clients under the Code of Professional Responsibility of New York State and Washington State.
Your initial in-person consultation with our attorney will be invoiced in accordance with our regular office procedures. The Firm accepts payment by check, cash or credit card (Visa or Master Card).
If there is an emergency which requires immediate action, please let us know when you schedule the consultation. Depending upon attorney availability and the nature of the emergency, we will do everything possible to try to accommodate your needs. Be prepared to explain the emergency situation, giving full names, dates, locations, telephone numbers, etc., and to immediately provide copies of relevant documents to us via facsimile at 425-209-0211, or via email to firstname.lastname@example.org.
